Effect of a structured eight-week pulmonary rehabilitation program on quality of life and exercise tolerance in patients with moderate chronic obstructive pulmonary disease

Abstract

Background: Chronic obstructive pulmonary disease (COPD) is a leading cause of disability worldwide, characterised by progressive airflow limitation, dyspnoea, and reduced exercise capacity. Pulmonary rehabilitation is widely recommended as a core component of COPD management, yet adherence and outcome data from structured programs remain variable across settings.

Objective: To evaluate the effect of an eight-week structured pulmonary rehabilitation program on health-related quality of life and exercise tolerance among patients with moderate COPD.

Method: This study uses simulated academic training data designed to resemble a prospective, single-group, pre–post interventional design. A hypothetical cohort of 60 patients with spirometry-confirmed moderate COPD (GOLD stage II) underwent a supervised eight-week program comprising aerobic exercise, resistance training, breathing exercises, and patient education. Outcomes were assessed using the St George's Respiratory Questionnaire (SGRQ) and the Six-Minute Walk Test (6MWT) at baseline and post-intervention.

Results: Simulated analysis demonstrated a mean reduction in SGRQ total score of 12.4 points and a mean increase in 6-minute walk distance of 48.6 metres following the program, both exceeding established minimal clinically important difference thresholds (p < 0.001).

Conclusion: Within this simulated training framework, structured pulmonary rehabilitation was associated with clinically meaningful improvements in quality of life and exercise tolerance, supporting findings reported in the published literature. Real-world prospective trials remain necessary to confirm these illustrative findings.
